A birth notification number and a copy of the birth notification are required for online application. If you do not have them and the child was born in Nairobi, then visit the civil registration office at Bishop House in Nairobi.
Once you register online for NHIF, wait for 60 days or until when you are entitled to be covered, then go to NHIF Upper Hill office if you are in Nairobi and they will print your NHIF card for free. Therefore, you do not need to apply for the card. The online NHIF registration is enough.
