What the requirements for replacing a lost NHIF Card?

To replace a lost NHIF Card, pay Ksh. 100 to NHIF bank account (KCB or Equity) and go with the deposit slip to NHIF desk huduma center or NHIF Upper Hill office, Nairobi. Your new card will be printed.
